LCS access request
When a user is asking for access to LCS, this is the form that they should use:

Before starting on the ticket the below approvers should have provided their approval in the Fresh Service ticket.
Approvers: Johanna Strand (BPM) and Peter Schlyter (BPO).
When access is approved the ticket should be assigned to D365 L2 team and Columbus agent. Access is added inside of the LCS system by some of the administrators.
